When Do You Have to Provide A Copy of the Medical Record Without Charge?

A health care provider or medical record company must provide one copy of an individual's medical record without charge to the following:

Ohio Bureau of Workers' Compensation
Ohio Industrial Commission
Ohio Department of Job and Family Services
A patient or patient's representative if the medical record is necessary to support a claim under Title II or Title XVI of the Social Security Act
